im extremely prone to heel blisters (i even get them with sneakers) and i have never had an issue with the back strap on my milanos. i dont do it up tight though, i keep it loose enough that it doesnt touch my skin but tight enough that it helps the sandals stay on when im walking if that makes sense. youd think you need the strap tight for it to work but it really helps the sandals stay on.
